Recently, I was asked to coach my daughter's softball team. The first thing I wanted to do was ask the players some questions about themselves so I created a small questionnaire for them to fill out during our parent meeting. So, now I'm making it available to you! This is my Baseball / Softball Player Questionnaire Template for Pages.

This questionnaire template is a simple half sheet of paper questionnaire (2 per page). It's really simple to edit how you want. Change the questions, change the font, add your team logo to the top, or edit it anyway you want consider this your starting point.
