This template has possibly been created for a paper size other than A4. However, with no notes, it isn’t possible to know that for certain. It certainly isn’t formatting well on my screen with A4 set as the default page size.
The next thing that’s bothering me is the use of tabs to seemingly move from one column to the next. I don’t understand why that would have been done. It means, as far as I understand things, that no font substitution can be made – type or size. And then using the space bar to line things up? Except that they don’t line up.
Next, the words, ‘home address’ appear underneath ‘Name’. Underneath, not as in distance from the top of the page, but in the sense of layers on a page. This could be related to the page size problem.
What are all of the # symbols all over the place for? I thought that this was an address book template, not a musical score.
And the blue highlighting is all over the place with what is highlighted. Or that’s how it shows on my system.
I could have modified some things, like the # everywhere. But the rest of the formatting errors on my system, unfortunately make it unusable for me.
